Returning Patient Forms

You can start filling out the Patient Forms on the right.  If you run into any problems completing the forms you can email us at PatientCare@piedmontdental.com or call/text us at (704) 225-8555​. 

 

We encourage all of our  patients to arrive 15 minutes early to their appointment. Please bring with you a photo ID for your records. 

 

Previous Records

If you have records from your previous office, bring them with you too. 

 

Special Needs / Medical 

Premedication: Patients with joint replacements (hip, knee, etc) will more than likely need to be premedicated prior to their dental visit. Please talk to your orthopedic doctor about your premedication requirements. And let us know if we need to call in a refill for you.

• A daytime telephone number along with the best time to call,

• Your insurance information, if any

• The reason for your visit 

• The day and time you would like to schedule an appointment. Our office hours are: 

 

Monday:        8:30 am – 5 pm

Tuesday:       8:30 am – 5 pm

Wednesday:  8:30 am – 5 pm

Thursday:      8:30 am – 3 pm

Friday and Saturday By Appointment Only*
